HRT in Malta: is it free, what it costs, how to get it

No — as of June 2026, menopausal HRT is not publicly funded in Malta. The Government Formulary covers these hormones only for named conditions (Turner syndrome, hypogonadism, hypopituitarism, malignant disease, transgender care) — menopause is not one of them — and the modern first-line products aren't listed at all. Whichever door you walk through, public or private, HRT for menopause is bought out-of-pocket. A government pledge to change this exists; it hasn't happened yet.

Is HRT free in Malta?

The public system will see you for free — a health-centre GP visit costs nothing for residents, and a GP referral to Mater Dei's Gynae Outpatients clinic is free too. But being seen and being treated are different things. The Pharmacy of Your Choice (POYC) scheme dispenses free medicines only from the Government Formulary List, and menopause is not a qualifying condition on it.

Specifically, as of the June 2026 formulary update:

• Hormone products on the list are restricted to named conditions: Turner syndrome, hypogonadism, hypopituitarism, malignant disease, and transgender care. Menopause does not qualify.
• The products modern guidelines reach for first — estradiol patches and gel, micronised progesterone, vaginal estrogen, tibolone — are not on the list for any condition.

So the practical answer is: the visit can be free; the medicine is not.

What about the election pledge?

On 13 May 2026, ahead of the general election, the Labour Party pledged that HRT and endometriosis medication would be added to the Government Formulary — that is, made free. Labour won the election on 30 May 2026. As of July 2026, the pledge has not yet appeared in the formulary list, which was last updated 16 June 2026.

How do I actually get HRT in Malta?

The pathway is short, but nobody hands it to you:

1. A doctor writes the prescription. That can be a health-centre GP, a private GP, or a gynaecologist. There is no dedicated public menopause clinic at Mater Dei or Gozo General — in the public system, menopause is handled inside general gynaecology, via GP referral. Most women who want HRT initiated use a private gynaecologist.
2. You fill it at a community pharmacy. Any pharmacy dispenses private prescriptions. Which brands and strengths are physically on the shelf varies pharmacy to pharmacy and week to week.
3. You phone ahead. HRT stock in Malta is volatile — shortages that hit Europe hit Malta. Call the pharmacy, name the exact product on your prescription, and confirm it's in stock before you travel.

How much does HRT cost in Malta?

Why can't I find estradiol patches right now?

2026 has an international shortage of estradiol (oestrogen) patches — demand surged and roughly one in two women internationally has struggled to fill a patch script. Malta imports its supply, so European swings land here.

If patches are out: oestrogen gel, spray, or oral tablets treat the same symptoms. Switching format is a routine prescriber conversation, not a setback. This is also exactly the situation where phoning three pharmacies beats visiting one.

What if my product isn't sold in Malta at all?

Malta has a formal route for medicines not marketed locally: the Exceptional Medicinal Treatment (named-patient) route, where a medical consultant submits an EMT Request Form to the Directorate for Pharmaceutical Affairs (legal basis: L.N. 58 of 2018). It's consultant-initiated — you can't file it yourself — but knowing it exists changes the conversation when a pharmacist says "we can't get that here."

Is HRT even right for me?

That depends almost entirely on which stage you're in — the evidence, the products and the trade-offs differ between early perimenopause, late perimenopause and postmenopause, and the calculation changes again for early menopause and POI, where NICE guidance treats hormone replacement as the default until the natural age of menopause, not an option.
