What menopause care actually costs in Malta

Here's the inversion nobody spells out: Malta's public system funds the visit, not the menopause medicine. Being seen is free for residents — the GP, the Mater Dei referral, doctor-ordered bloods. But menopausal HRT is not on the June 2026 Government Formulary, so you buy it out-of-pocket whichever door you came through. Private consults are typically quoted around €50–80; blood panels and follow-ups are quoted at booking, not published. A government pledge to make HRT free exists — it hasn't happened yet. What does the public system actually pay for?

The visit — not the medicine. That one sentence is the whole story, and it catches women off guard because "the public system covers it" sounds like it should cover the treatment too. It covers being seen:

• A health-centre GP visit is free for residents.
• A GP referral to Mater Dei Gynae Outpatients is free or subsidised for residents.
• Doctor-ordered blood tests are free through health-centre and Mater Dei phlebotomy.

What it does not pay for is the HRT itself. The Pharmacy of Your Choice (POYC) scheme dispenses free medicines only from the Government Formulary List — and as of the June 2026 update, the hormones on that list are restricted to named conditions (Turner syndrome, hypogonadism, hypopituitarism, malignant disease, transgender care), with menopause not among them. The modern first-line products — estradiol patches and gel, micronised progesterone, vaginal estrogen, tibolone — aren't listed at all. Read the bottom row twice. The HRT line is out-of-pocket in both columns — that's the inversion in a single row. The public column saves you the consultation and testing costs; it does not save you the medicine cost. Why are the prices "quoted, not published"?

Because that's how the Maltese private market works — it's a structural fact, not an oversight to route around. Clinics and labs give you the figure when you book rather than posting a rate card online. It has two consequences worth internalising:

• Any price you read is a planning anchor, not a quote — including every figure on this page. Phone to confirm the current number before you commit.
• We never invent a precise figure. Where a real range exists we give it; where it doesn't, we say "quoted at booking" and tell you to call. A made-up "€120 panel" would read as precise and be worthless.

The one fact to watch: the formulary pledge

On 13 May 2026, ahead of the general election, the Labour Party pledged to add HRT and endometriosis medication to the Government Formulary — making them free — alongside a clinic focused on women's healthcare. Labour was re-elected on 30 May 2026. As of July 2026, the pledge has not yet appeared in the formulary, which was last updated 16 June 2026.

This is the single most changeable fact on this page, so it's worth knowing exactly what would change if it's implemented.

What changes if the formulary adds HRT?

One row, and it's the important one. If menopausal HRT is added to the Government Formulary, the medicine moves from out-of-pocket to free at point of access through the POYC scheme for eligible residents. The rest of the map barely shifts:

• The medicine cost drops — potentially to nothing, via POYC, for the formulary products.
• The visit stays as it is — free on the public pathway, quoted privately.
• Private consults and panels stay quoted, not published — a formulary change doesn't alter how private clinics price.
